Summary
We are seeking to add caring, compassionate Occupational Therapists to our growing team. Our Physical Therapists will deliver individualized, evidence-based therapy to support patient recovery and maximize mobility. They will evaluate patient needs, develop individualized treatment plans, and implement therapeutic activities tailored to each patient’s needs and environment. This role involves close collaboration with patients, families, and interdisciplinary teams to ensure safe, effective, and goal-oriented care. 

Key Responsibilities:

  • Patient Assessment and Evaluation – Conduct comprehensive assessments of patients’ physical, cognitive, and emotional abilities to determine functional limitations and care needs.
  • Treatment Planning and Implementation – Develop and execute individualized therapy plans focused on improving patients’ independence in activities of daily living (ADLs) and home safety.
  • Patient and Caregiver Education – Train patients and caregivers on adaptive techniques, use of assistive devices, and home safety modifications to promote self-sufficiency.
  • Interdisciplinary Collaboration – Coordinate care and communicate effectively with physicians, nurses, physical therapists, and other team members to ensure integrated treatment.
  • Documentation and Progress Reporting – Maintain accurate, timely clinical records and report patient progress, treatment outcomes, and recommendations in compliance with regulatory and agency


What We’re Looking For

  • Current license in the state of Colorado
  • At least 1 year of home health care Occupational Therapy experience
  • Strong interpersonal and critical thinking skills
  • Compassionate, patient-centered mindset
  • Reliable transportation and valid driver’s license
