Schedule: Monday–Friday, 8:00 AM–4:30 PM

Weekly Guarantee:40 hours

Mileage Reimbursement: Yes, reimbursed between visits, from first patient to last patient

Dress Code: Navy scrubs, closed‑toe shoes 

License Required: Alabama Occupational Therapy License

 

Position Overview

We are seeking a dedicated Occupational Therapist to join our Home Health team serving Eufaula, AL. This role is ideal for a clinician who values autonomy, one‑on‑one patient care, and the opportunity to support patients in their home environments.

 

Primary Responsibilities

• Patient Evaluation: Assess functional status and determine occupational therapy needs; collaborate with the physician to develop the plan of care.
• Therapy Delivery: Provide treatment within Alabama OT practice laws, following the plan of care and coordinating with the interdisciplinary team.
• Condition Monitoring: Observe and report patient responses and changes in condition to the supervising nurse and/or physician.
• Caregiver and Team Education: Educate patients, families, and care team members on the plan of care, functional limitations, and progress.
• Home Visits and Documentation: Complete visits per the treatment plan, submit progress notes per policy, and maintain accurate, timely documentation.
• Plan of Care Development: Complete evaluations and develop OT plans of care within state guidelines.
• Outcome Reporting: Communicate evaluation results, goals, and projected frequency of care.
• Adaptability: Provide care in diverse home environments regardless of patient background or circumstances.

 

Required Qualifications

• Current Occupational Therapist Licensure in Alabama
• CPR Certification
