Job description:

We are seeking a compassionate Occupational Therapist (OT) to join our growing Home Health team serving San Joaquin, Stanislaus, and Merced Counties. You'll provide one-on-one care in patients' homes, helping them regain independence and improve their quality of life through personalized treatment plans.

Employment type - Part-Time & Per Diem

Pay: $60.00 $65.00 per hour

Service Areas: -

• North / San Joaquin County - Stockton and Salida
• Central / Stanislaus County - Modesto and Oakdale
• South / Merced County - Merced, Los Banos and Salida

Responsibilities

• Evaluate patients' functional abilities and home environment.
• Develop and implement individualized treatment plans.
• Provide skilled occupational therapy services in the home.
• Educate patients, families, and caregivers on treatment plans and adaptive techniques.
• Recommend adaptive equipment and home safety modifications.
• Complete timely and accurate clinical documentation.
• Collaborate with physicians and the interdisciplinary care team to ensure quality patient care.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current California Occupational Therapist license.
• Minimum of 2 years of clinical OT experience.
• Home Health experience preferred.
• Current CPR certification.
• Excellent communication and organizational skills.
• Valid driver's license and reliable transportation.
