Job description:
We are seeking a compassionate Occupational Therapist (OT) to join our growing Home Health team serving San Joaquin, Stanislaus, and Merced Counties. You'll provide one-on-one care in patients' homes, helping them regain independence and improve their quality of life through personalized treatment plans.
Employment type - Part-Time & Per Diem
Pay: $60.00 $65.00 per hour

- North / San Joaquin County - Stockton and Salida
- Central / Stanislaus County - Modesto and Oakdale
- South / Merced County - Merced, Los Banos and Salida

Responsibilities
- Evaluate patients' functional abilities and home environment.
- Develop and implement individualized treatment plans.
- Provide skilled occupational therapy services in the home.
- Educate patients, families, and caregivers on treatment plans and adaptive techniques.
- Recommend adaptive equipment and home safety modifications.
- Complete timely and accurate clinical documentation.
- Collaborate with physicians and the interdisciplinary care team to ensure quality patient care.
Qualifications
- Graduate of an accredited Occupational Therapy program.
- Current California Occupational Therapist license.
- Minimum of 2 years of clinical OT experience.
- Home Health experience preferred.
- Current CPR certification.
- Excellent communication and organizational skills.
- Valid driver's license and reliable transportation.
