About the Role: Join CARE ESSENTIALS HOME HEALTH as a Home Health Occupational Therapist in San Bernadino, AL, where you will play a vital role in enhancing the quality of life for our patients. Responsibilities:
Evaluate patients’ needs and develop personalized treatment plans.
Implement therapeutic interventions to improve patients' daily living skills.
Monitor patient progress and adjust treatment plans as necessary.
Collaborate with healthcare professionals to ensure comprehensive patient care.
Educate patients and their families on occupational therapy processes.
Maintain accurate documentation of patient care and progress.
Provide support and guidance to patients in their recovery journey.
Stay updated with the latest occupational therapy practices and regulations.
Requirements:
Master's degree in Occupational Therapy from an accredited institution.
Current state licensure to practice as an Occupational Therapist.
Strong communication and interpersonal skills.
Experience in home health care preferred.
Ability to work independently and as part of a team.
Compassionate demeanor with a patient-centered approach.
Solid organizational skills and attention to detail.
Willingness to travel to patients' homes as needed.
