About the Role

We’re a Medicare‑certified, Pennsylvania‑licensed home health agency seeking a PRN Occupational Therapist to deliver skilled, in‑home therapy services across the greater Philadelphia region. You’ll support patients recovering from surgery, hospitalization, illness, injury, or disability, helping them regain independence and safely function in their home environment.

What You’ll Do

• Conduct comprehensive occupational therapy evaluations in patient homes

• Develop and implement individualized treatment plans based on physician orders

• Provide interventions focused on ADLs, functional mobility, coordination, strength, and safety

• Assess home environments and recommend adaptive equipment or modifications

• Educate patients and caregivers on adaptive techniques, fall prevention, and home safety

• Track progress and adjust treatment plans as needed

• Collaborate with RNs, PTs, SLPs, and social workers to coordinate care
