    Home Health Occupational Therapist

    Full Time/ Plymouth Territory

    Elara Caring is seeking a compassionate Home Health Occupational Therapist who wants to provide meaningful, one-on-one care where patients feel most comfortable: at home. You’ll help people rebuild essential skills, regain confidence, and become more independent after an illness, injury, or hospitalization.

    What You’ll Do

    • Evaluate patients’ functional abilities and rehabilitation needs

    • Develop personalized plans of care focused on safety and independence

    • Help patients improve daily activities such as dressing, bathing, cooking, and mobility

    • Teach patients and caregivers how to safely use adaptive equipment

    • Recommend equipment and home modifications that make daily life easier

    • Monitor progress and adjust treatment plans as patient needs change

    • Communicate important updates to physicians and other members of the care team

    • Coordinate safe and effective discharge planning

    • Provide clinical direction to Certified Occupational Therapy Assistants and other care team members

    What You’ll Need

    • Graduate of an accredited Occupational Therapy program

    • Current, unrestricted Occupational Therapist license in the state of practice

    • At least one year of clinical care experience

    • Ability to travel throughout the assigned territory

    • Dependable vehicle, valid driver’s license, and current automobile insurance

    • Ability to travel up to 50%

    • Flexibility to work varying or on-call hours when applicable

    • Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
