We are seeking a compassionate and motivated Occupational Therapist (OT) to join our growing Home Health team. The ideal candidate is patient-focused, organized, and passionate about helping individuals maintain independence and quality of life in their homes.
Position Type: Full-time / Part-time / PRN
Setting: Home Health
Responsibilities:
- Evaluate patients in their homes and develop individualized treatment plans
- Provide skilled OT services to improve daily living skills and functional independence
- Educate patients and caregivers on adaptive techniques and safety
- Document evaluations, progress notes, and discharge summaries in a timely manner
- Collaborate with interdisciplinary healthcare team members
- Follow all state, federal, and agency regulations
Requirements:
- Current Occupational Therapist license in good standing
- Graduate of an accredited OT program
- Home health experience preferred (but not required)
- Reliable transportation and valid driver’s license
- Strong communication and time-management skills
- Ability to work independently
