Responsibilities:
Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist
Implement individualized treatment plans focused on mobility, strength, balance, gait training, and functional independence
Monitor patient progress and communicate changes or concerns to the supervising Physical Therapist and interdisciplinary care team
Educate patients and caregivers on exercises, safety precautions, and home programs
Maintain accurate clinical documentation and comply with all regulatory and agency requirements
