Travel Physical Therapist – Home Health

Location: Cape Coral & North Fort Myers, FL

Position Overview

Mederi Caretenders is seeking an experienced Travel Physical Therapist (PT) to join our Home Health team serving Cape Coral and North Fort Myers. This position is ideal for a clinician who is patient-focused, organized, and thrives in an autonomous home health environment.


Schedule & Commitment

  • Weekly Hour Guarantee: 40 hours
  • Schedule: Monday–Friday, 8:00 AM – 4:30 PM
  • On-Call/Weekend Requirement: None

Coverage Area

  • Cape Coral
  • North Fort Myers

Mileage is reimbursed between patient visits, from the first patient to the last patient of the day.


Key Responsibilities

The Physical Therapist will be responsible for delivering high-quality home health services, including but not limited to:

  • Completing Start of Care (SOC) assessments
  • Performing initial evaluations and ongoing skilled therapy visits
  • Conducting recertifications and reassessments
  • Completing discharge visits with accurate clinical documentation
  • Developing and updating individualized care plans
  • Collaborating with the interdisciplinary care team to ensure continuity of care
  • Maintaining compliance with agency policies and regulatory standards

Required Qualifications

  • Active Physical Therapist (PT) license
  • Home Health Physical Therapy experience (required)
  • Ability to work independently in a home-based care environment
  • Strong clinical documentation and time-management skills
