Home Health & Hospice Physical Therapist (Contract)
Full-Time | 40 Hours Guaranteed Weekly
Coverage Areas: Madison, Montgomery, Clark, Powell Counties
Pay Rate: $50/hr - $53/hr
Schedule & Coverage
- Shift: Monday–Friday, 8:00 AM–4:30 PM EST
- On-Call Requirements:
- 1 weekend per month
- 1 weekday per week
About the Role
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our Home Health & Hospice team. This role offers autonomy, meaningful patient relationships, and the opportunity to make a direct impact on patients’ quality of life in their homes. Key Responsibilities
- Start of Care (SOC) assessments
- Comprehensive evaluations and individualized care planning
- Recertifications and ongoing reassessments
- Resumption of Care (ROC) visits
- Agency discharges within PT scope of practice
- Collaborating with interdisciplinary team members to ensure coordinated, patient-centered care
- Documenting timely and accurately in accordance with regulatory and agency standards
Required Qualifications
- Active Physical Therapist (PT) license in the state
- OASIS proficiency
- Experience with HCHB/PointCare or similar EMR systems
- Current BLS certification
Preferred Experience (Nice-to-Haves)
- Prior Home Health experience
- Strong clinical judgment and autonomous decision-making
- Excellent communication and time‑management skills
- Ability to adapt to changing patient needs and home environments
