Pride Health is hiring a Home Health Physical Therapist to support our clients home health patient population across Wabsh, Huntington, and Grant County, IN. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization!

Location: Marion, IN 46952
Coverage Area: Wabsh/Huntington/Grant County
May also support patients in Blackford, Delaware, and Madison Counties (as needed)
Job Type: Contract
Shift: Monday-Friday 8:00 am to 4:30 pm (40 hrs/week)
Weekend Rotation: Every 3rd Saturday
Pay range: $54 to $56/hr on W2
Mileage reimbursement provided (from first patient to last patient visit)
Key Responsibilities:
Conduct comprehensive patient evaluations and develop individualized physical therapy plans of care in accordance with state guidelines
Perform home health visits, including:
Start of Care (SOC)
Resumption of Care (ROC)
Evaluations
Discharges
Deliver skilled physical therapy interventions to patients in their homes
Complete all clinical documentation within 24 hours of each visit
Participate in interdisciplinary team meetings, case conferences, and branch meetings
Maintain flexibility in scheduling to meet patient care needs
Participate in weekend rotation and be available for occasional holiday coverage
