Pride Health is hiring a Home Health Physical Therapist to support our clients home health patient population across Wabsh, Huntington, and Grant County, IN. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization!

Job Details:

  • Location: Marion, IN 46952

  • Coverage Area: Wabsh/Huntington/Grant County

  • May also support patients in Blackford, Delaware, and Madison Counties (as needed)

  • Job Type: Contract

  • Shift: Monday-Friday 8:00 am to 4:30 pm (40 hrs/week)

  • Weekend Rotation: Every 3rd Saturday

  • Pay range: $54 to $56/hr on W2

  • Mileage reimbursement provided (from first patient to last patient visit)

Key Responsibilities:

  • Conduct comprehensive patient evaluations and develop individualized physical therapy plans of care in accordance with state guidelines

  • Perform home health visits, including:

  • Start of Care (SOC)

  • Resumption of Care (ROC)

  • Evaluations

  • Discharges

  • Deliver skilled physical therapy interventions to patients in their homes

  • Complete all clinical documentation within 24 hours of each visit

  • Participate in interdisciplinary team meetings, case conferences, and branch meetings

  • Maintain flexibility in scheduling to meet patient care needs

  • Participate in weekend rotation and be available for occasional holiday coverage

Required Qualifications:

  • Current, unencumbered Physical Therapy license in the state of Indiana
  • AHA BLS certification required
  • Valid driver's license, auto insurance, and access to a reliable vehicle
  • Previous home health experience with understanding of Medicare guidelines
  • HomeCare HomeBase (HCHB) software experience preferred.
