Home Health Physical Therapist - Michigan

Job Summary

The Physical Therapist reports to the Director of Professional Services and is responsible for providing medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, company policies, and professional Standards of Practice.

Essential Functions of Position

• Participate in the development of patient physical therapy plans of care.
• Conduct patient assessments and evaluations, communicating with the physician as needed before the start of care.
• Implement the plan of care, document the care provided, and submit paperwork accurately and promptly to maintain patient medical records.
• Provide education and instruction to team members, patients, and their families.
• Participate in agency quality assessment performance improvement (QAPI) programs as needed.
• Ensure the growth and profitability of the company through responsible use of company resources and community education regarding services.
• Attend regularly scheduled interdisciplinary team meetings to coordinate patient and family care, exchange information, problem-solve, and receive staff support and education.
• Apply agency policies and procedures, as well as federal and state regulatory agency rules and regulations, in providing care.
• Provide skilled restorative treatments for pain reduction and develop/restore/maintain functions using various therapeutic techniques.
• Conduct initial and ongoing comprehensive assessments of patient needs, including outcome and information set (OASIS) assessments at appropriate time points.
• Evaluate outcomes of care.
• Direct and supervise Physical Therapist Assistants, Certified Nursing Aides, and Home Health Aides in patient care, providing supervisory visits as necessary.
• Accept direction from the Director of Professional Services, Patient Care Coordinator, or Rehabilitation Supervisor.

Required Skills & Experience

• Knowledge of principles, concepts, and methodology of physical therapy appropriate for terminally ill patients.
• Demonstrated problem-solving and decision-making abilities.
• Ability to manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention, and family intervention.
• Excellent communication skills.
• Ability to develop and maintain rapport with patients, families, and staff.

Preferred Skills & Experience

• One year or more work experience in physical therapy in a multidisciplinary setting is preferred.

Schedule Notes

Monday-Friday 8a-5p. No call. 60-mile travel radius. HCHB required. Estimated 6 patients per day.
