Washington, PA – Casual; Hourly, Non-exempt 

 

Starting Rate: Pay is by Productivity Point, based on experience

Presbyterian SeniorCare Network Home Health is a Medicare-certified home health agency that provides in-home care and services when a physician prescribes for them. Managed by Redstone Presbyterian SeniorCare, our home health professionals make a difference in the lives of older adults by providing skilled nursing services; and physical, occupational, and speech therapy while our patients remain in a comfortable, at-home setting.

 

The Physical Therapist – Home Health delivers care to patients in their place of residence in accordance with the patient’s individualized plan of care through the process of data collection, planning, implementation and evaluation and works effectively and interacts with the patients, caregivers, and other members of the Presbyterian SeniorCare at Home team while maintaining quality care in accordance with accepted standards of practice.

Requirements for thePhysical Therapist – Home Health 

 

  • Bachelor’s or master’s degree in physical therapy approved by an accredited organization: The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, The Council on Medical Education of the American Medical Association, or The American Physical Therapy Association. 
  • Minimum of 2 years as a physical therapist.  
  • Current CPR Certification. 
  • Valid Driver’s License and reliable transportation that is insured in accordance with PA requirements. 
  • Maintain a professional attitude and demeanor and communicate effectively and relate courteously and cooperatively with patients, caregivers, colleagues, supervisors, and all others. 
  • Demonstrate optimism, enthusiasm, and willingness to work constructively with other health care team members and reflect concern for the well-being of patients and others. 

 

Community/Home Health experience is preferred.
