Home Health Physical Therapy Assistant

About the Role:
Trust Home Care Inc is looking for a dedicated and compassionate Home Health Physical Therapy Assistant to join our growing team in  Harford County. In this rewarding role, you'll make a meaningful difference by delivering hands-on rehabilitative care directly to patients in the comfort of their own homes. Responsibilities:

• Implement physical therapy treatment plans under the supervision of a licensed Physical Therapist
• Provide in-home therapeutic exercises, gait training, and functional mobility assistance
• Monitor and document patient progress, reporting changes in condition to the supervising PT
• Educate patients and family members on home exercise programs and safety techniques
• Assist patients with activities of daily living (ADLs) to promote independence and recovery
• Maintain accurate and timely clinical documentation in compliance with home health regulations
• Coordinate care with interdisciplinary home health team members to ensure continuity of care

Requirements:

• Active Maryland PTA license in good standing
• Graduate of an accredited Physical Therapist Assistant program
• Previous home health or outpatient PT experience preferred
• Valid driver's license and reliable transportation for home visits in the Bel Air, MD area
• Strong knowledge of rehabilitative techniques, therapeutic exercise, and patient safety
• Excellent communication, time management, and organizational skills
• Compassionate, patient-centered approach with a commitment to quality care
