Summary:

  • To provide physical therapy treatment, under the directions and supervision of a licensed physical therapist, to patients receiving home care services through Penn Medicine at Home (PMAH).

Responsibilities:

  • Receive patient clinical and scheduling information through the supervising PT or team scheduler

  • Review and follow the PT treatment plan and individual patient goals. Be aware of any precautions set forth by the physician or supervising PT

  • Correctly interpret the instructions for patient treatment set forth by the physical therapist. Appropriately advance the home exercise program and therapeutic activities to improve function.

  • Display compassion and empathy in all patient interactions. Maintain an accurate schedule on the documentation software.

  • Documents all contact with patient/family/payors or referral sources in road notes.

  • Specific knowledge of other therapy disciplines and services provided (occupational, and speech therapy).

  • Demonstrates proficiency in required software systems including road notes and outlook.

Credentials:

  • Basic Cardiac Life Support (Required)
  • Car Insurance (Required)
  • Driver's License (Required)
  • Physical Therapy Assistant (Required)

Education or Equivalent Experience:

  • Associate of Science (Required)
  • Education Specialization: CAPTE certified physical therapy assistant program
