The Role
As a PRN PT with StellarCare, you will provide
high-quality home health services to patients, helping them regain
independence where they feel most comfortable. You pick the coverage
areas and the days you want to work—we handle the rest.
What You’ll Do
Restore Independence: Conduct initial evaluations and develop
functional, goal-oriented care plans.
Clinical Excellence: Provide one-on-one therapeutic interventions,
including gait training, strengthening, and manual therapy.
Safety First: Perform home safety assessments and recommend
modifications to prevent falls.
Documentation: Complete timely OASIS assessments and progress
notes via our intuitive EMR system.
Collaborate: Work closely with physicians and the
StellarCare clinical team to ensure seamless patient transitions.

Requirements
What You Bring
Credentials: A degree in Physical Therapy from an
accredited program and a valid state license.
Experience: Minimum of 1 year of clinical experience
(Home Health experience preferred).
Reliability: A valid driver’s license, auto insurance, and
dependable transportation.
Tech-Savvy: Comfort with electronic documentation and
mobile communication.
