Physical Therapist (PT) / Physical Therapist Assistant (PTA)
Responsibilities:
- Evaluate patients' mobility, strength, and functional abilities
- Develop and implement individualized treatment plans in collaboration with physicians
- Provide skilled therapy interventions and adjust treatment plans as needed
- Monitor patient progress and outcomes
- Educate patients and caregivers on therapy goals and home exercise programs
- Complete accurate and timely documentation (Axxess experience preferred)
PT/PTA Requirements:
- Valid Oklahoma PT or PTA license
- CPR Certification
- Reliable transportation and valid Oklahoma driver's license
- Strong communication and documentation skills
- Home health experience
