Core Responsibilities
  • Conduct comprehensive initial and ongoing nursing assessments for patients across a defined service territory
  • Develop, implement, monitor, and regularly update individualized patient care plans in collaboration with physicians and interdisciplinary team members
  • Administer medications, wound care, injections, IV therapy, and other skilled nursing interventions as ordered
  • Educate patients and their family members on disease management, medication adherence, safety practices, and self-care techniques
  • Monitor patient progress closely and communicate changes in condition promptly to the supervising physician and care team
  • Maintain accurate, timely, and compliant clinical documentation using electronic health record systems
  • Coordinate seamlessly with physical therapists, occupational therapists, social workers, and other healthcare professionals to ensure comprehensive, continuous patient care
  • Participate in quality improvement initiatives and maintain compliance with all relevant regulatory standards and agency policies
  • Respond to urgent patient needs and provide telephone triage support when required

Must-Have Requirements
  • Active Registered Nurse license in good standing
  • Minimum of one year of clinical nursing experience, with home health, medical-surgical, or critical care experience strongly preferred
  • Current Basic Life Support certification required
  • Valid driver's license and reliable personal transportation for patient home visits
  • Proficiency with electronic health record documentation systems
  • Strong organizational, time management, and prioritization skills
  • Excellent interpersonal communication and patient education abilities
  • Ability to work both independently and collaboratively within a multidisciplinary team environment
