Home Health Registered Nurse

About the Role
Anuco Home Health Care LLC is seeking compassionate, dependable, and highly skilled Registered Nurses to provide intermittent skilled nursing services in patients’ homes. The Registered Nurse will assess patient needs, develop and coordinate individualized plans of care, perform skilled nursing procedures, supervise assigned clinical personnel, and help patients safely manage their health at home.
The ideal candidate has Medicare-certified home health experience, strong OASIS and care-coordination skills, proficiency with Axxess Home Health EMR, and a consistent record of accurate and timely documentation.
Responsibilities
- Complete initial and ongoing comprehensive nursing assessments.
- Perform Start of Care, Resumption of Care, Recertification, Transfer, Discharge, and other OASIS assessments as assigned.
- Develop, implement, evaluate, and revise individualized plans of care.
- Provide skilled nursing procedures, treatments, medication management, disease education, and preventive or rehabilitative nursing care.
- Evaluate the patient’s response to treatment and continued need for skilled services.
- Process physician orders and obtain clarification or updated orders when needed.
- Notify the physician, Clinical Manager, and interdisciplinary team promptly of changes in condition, hospitalizations, falls, medication concerns, incidents, or unmet needs.
- Coordinate services with PT, OT, SLP, MSW, HHA, physicians, caregivers, and other providers.
- Educate patients and caregivers regarding medications, diagnoses, treatments, safety, emergency planning, and self-management.
- Complete recertification documentation, care summaries, and discharge summaries.
- Supervise assigned LPNs and Home Health Aides according to agency and regulatory requirements.
- Participate in case conferences, staff meetings, medical-record reviews, QAPI activities, and required training.
- Provide on-call, weekend, or holiday coverage when assigned.
- Protect patient confidentiality and comply with HIPAA and agency policies.
Axxess and Documentation Expectations
- Complete all visit documentation accurately in Axxess within agency-required timeframes.
- Ensure documentation supports the skilled service provided, patient response, progress toward goals, homebound status, and continued need for care.
- Complete OASIS assessments, medication profiles, plans of care, orders, coordination notes, wound documentation, and discharge records accurately.
- Confirm that visit frequency, physician orders, interventions, and documentation are consistent.
- Respond promptly to QA corrections and return corrected documentation within the requested timeframe.
- Notify the office immediately if a clinical, scheduling, or technical issue may delay documentation.
- Understand that incomplete or late documentation may delay care coordination, authorization, billing, and future assignments.
Requirements
- Current, unrestricted Illinois Registered Nurse license.
- Graduate of an approved professional nursing program.
- Minimum of one year of nursing experience preferred.
- Medicare-certified home health experience strongly preferred.
- OASIS experience strongly preferred.
- Proficiency with Axxess Home Health EMR preferred.
- Current CPR or BLS certification.
- Valid driver’s license, reliable transportation, and current automobile insurance.
- Professional liability insurance as required.
- Strong assessment, critical-thinking, communication, and time-management skills.
- Ability to work independently and travel to patient homes.
- Ability to perform essential job duties with or without reasonable accommodation.
Clinical Territories
Applicants may select one or more territories they can cover consistently.
East/Central Territory: Chicago, Oak Park, River Forest, Elmwood Park, and Cicero.
Territory assignments depend on patient location, clinician availability, payer requirements, clinical competency, and agency needs.
