Your Role:
Conduct in-home patient assessments and develop individualized care plans.
Administer medications, treatments, and wound care as prescribed.
Educate patients and families on managing health conditions and promoting independence.
Monitor patient progress and update care plans as needed.
Supervise LPNs and Home Health Aides to ensure quality care.
Participate in interdisciplinary meetings to coordinate comprehensive care.
What You Need:
Active Registered Nurse (RN) license in the State of Arizona.
Minimum of one year of nursing experience (home health, hospice, or critical care preferred).
CPR certification and valid driver’s license with reliable transportation.
Excellent communication, organizational, and interpersonal skills.
