Active Home Health is seeking a PRN Home Health RN to serve patients throughout Utah County. Job Requirements:
Visit and assess patients to complete an initial comprehensive OASIS or assessment and ongoing assessments through end of care.
Chart all clinical documentation in our EHR software
Create and manage patient plan of care
Educate and give instruction to patient, family, and caregivers as appropriate per patient care plan.
Case manage the patient episode or benefit period, ensuring that all cares are performed and documented
Provides medication education and management per physician order
Works hand-in-hand with the IDT to ensure patient goals are met and appropriate outcomes are achieved
Shares on-call duties
Maintains strong communication with patient, family, caregivers, and the interdisciplinary teams
Job Qualifications:
Registered nurse with current license to practice professional nursing in the state of Utah
Polite and compassionate
Excellent observation, problem solving, verbal and written communications
Maintains a current CPR certification
1 year Home Health or Hospice experience preferred
Must be a licensed driver with an automobile that is insured, registered and in good working order
