This role provides essential hands-on support to patients in their homes, helping them maintain comfort, independence, and safety. Ideal candidates are compassionate, reliable, and committed to delivering high-quality care.Key ResponsibilitiesDeliver skilled nursing care according to individualized care plansPerform comprehensive assessments and ongoing patient evaluationsAdminister medications, wound care, injections, and other clinical treatmentsEducate patients and families on disease management, safety, and wellnessCollaborate with physicians, therapists, and the broader care teamDocument all care accurately and timely in accordance with regulatory standardsPromote patient independence and support positive health outcomesQualificationsExperience providing trach and respiratory care with precision and safety (Preferred)Experience with using a ventilator (Preferred)Experience with Infusions (Preferred)Current unencumbered RN license in the State of WisconsinBachelor's degree in nursing (Preferred)Certified CPR and a negative TB skin test or chest x-rayMinimum one (1) year of documented experience required in a home health agency, primary care clinic, or health facility (Preferred)Possess exceptional nursing assessment skillsAnalytic/Critical thinkerStrong clinical skillsLicensed driver with valid automobile insurance and reliable transportationMust be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. Varied assistance is required depending on the client's mobility.
