Pay: $56.14/hour. Mileage reimbursement available.
Schedule: Monday-Friday, 8:00am-5:00pm, no on-call

• Productivity expectation: 32 points per week
• Typical volume: 5-6 patients per day / 25-30 patients per week
  
  

Overview

We are hiring a Home Health RN in Gastonia, NC . This is an opportunity supporting patients in the home health settings. This role is ideal for an experienced Registered Nurse with strong case management and home health experience who is comfortable working independently in the field while coordinating care for high-risk members.

Key Responsibilities

• Perform comprehensive assessments of high-risk patients in the home health setting
• Collaborate with primary care providers to develop and implement individualized care plans
• Reassess patient needs and help ensure cost-effective, appropriate levels of care
• Coordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionals
• Arrange non-medical support services, including housing and transportation resources when needed
• Maintain detailed documentation of clinical, functional, and fiscal outcomes
• Identify opportunities for health promotion and illness prevention

Required Skills & Qualifications

• Active, unrestricted North Carolina RN license
• Minimum 1 year of home health experience
• Experience with wound care, wound vacs, infusion therapy, and PICC lines
• Experience with Homecare Homebase (HCHB) & OASIS
• Strong assessment, care coordination, and clinical decision-making skills

Preferred Experience

• Case management certification
