Responsibilities:
- Conduct comprehensive assessments of patients' health and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' progress and adjust care plans as necessary.
- Educate patients and their families on health management and disease prevention.
- Collaborate with interdisciplinary teams to ensure holistic patient care.
- Document all patient interactions and care provided in compliance with regulations.
- Provide emotional support and counseling to patients and families.
- Participate in ongoing training and professional development opportunities.
Requirements:
- Active Registered Nurse (RN) license in California.
- Minimum of 1 year of nursing experience, preferably in home health or community settings.
- Strong clinical skills and knowledge of patient care practices.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Compassionate demeanor with a focus on patient-centered care.
- Basic Life Support (BLS) certification required.
- Familiarity with electronic health records (EHR) systems is a plus.
