Well established home care agency has a need for an experienced Home Care RN Case Manager to see patients in homes in a coverage territory to include the North Attleboro, Foxboro,Bellingham, Franklin, and Mansfield, area.
Full-time, Monday-Friday, 40 hours/week
Shorter works weeks considered.
No weekends or on-call required!
Mileage reimbursement
The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
***Requires 1 year recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.****
