Well established home care agency has a need for an experienced Home Health RN Case Manager to see patients in homes in a coverage territory to include the Norwood/Westwood, MA area and surrounding towns.
Monday-Friday, 40 hours/week
No weekends or holidays!
Occasional on-call as needed
Highly competitive pay rate plus IRS mileage reimbursement
The Home Health Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
Requires minimum 1 year of recent Home Health RN Case Management experience doing OASIS visits with a Medicare certified agency.
