RN Case Manager

Salary: $79,000–$83,000 annually Full-Time | Field-Based Home Health Position | Limited On-Call

Spoon River Home Health is looking for a compassionate and dependable Registered Nurse (RN) Case Manager to join our team. This is a hands-on, field-based nursing position providing skilled care to patients in their homes while managing and coordinating their overall plan of care.

Our RN Case Managers work independently in the field while collaborating closely with physicians, supervisors, therapists, and other members of the interdisciplinary care team to help patients safely remain at home.

Responsibilities:

· Travel to patients' homes to provide skilled nursing care

· Complete patient assessments and ongoing evaluations

· Develop, implement, and update individualized plans of care

· Perform skilled nursing visits based on patient needs and physician orders

· Monitor changes in patient condition and communicate with physicians and the care team

· Educate patients and families about medications, diagnoses, treatments, and care needs

· Coordinate services with therapists, caregivers, physicians, and other healthcare professionals

· Complete timely and accurate clinical documentation

· Help ensure continuity, quality, and safety of care throughout the patient's home health experience

· Follow agency policies, clinical standards, and regulatory requirements

Qualifications:

· Current RN license (Illinois)

· Minimum of 1 year nursing experience

· Strong time management and organizational skills

· Dependable and self-motivated

· Committed to delivering compassionate, person-centered care

· Home health experience preferred, but not required
