We are seeking a compassionate, organized, and skilled Registered Nurse (RN) to join our team as an RN Case Manager. In this dual-faceted role, you will oversee clinical case management while directly providing high-quality skilled nursing care to seniors in the comfort of their homes.
A significant portion of our clients are honored military veterans. The ideal candidate will have a deep respect for those who served and a commitment to helping them maintain dignity and independence at home.
Key Responsibilities:
Case Management & Compliance: Develop, implement, and update comprehensive Plans of Care (POC). Ensure all clinical documentation complies with state, federal, and agency guidelines.
Clinical Supervision: Conduct supervisory visits for Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) to ensure top-tier care delivery.
Client & Caregiver Relations: Act as the primary point of contact for client and caregiver concerns, addressing issues promptly to ensure high satisfaction and safety.
Direct Skilled Care: Perform skilled nursing assessments and interventions for senior clients (wound care, medication management, teaching, etc.).
Utilization Review: Efficiently navigate and manage patient charts using Axxess software.
Qualifications & Requirements:
License: Active, unencumbered Registered Nurse (RN) license in the state of [Insert State].
Experience: Minimum of 1–2 years of experience working within a Medicare-certified Home Health Agency.
Software Proficiency: Strong, hands-on experience using Axxess home health software is highly preferred/required.
Clinical Skills: Confident in physical assessments, care planning, and managing chronic conditions common in the geriatric population.
Vehicle: Reliable transportation, valid driver’s license, and active auto insurance for traveling to client homes.
Soft Skills: Excellent communication, conflict resolution skills, and a heartfelt appreciation for veterans.
