Well established home care agency has a need for an experienced Home Care RN Case Manager to see patients in homes in a coverage territory to include the Waltham/Arlington/Lexington area.
Full-time, Monday-Friday, 40 hours/week
No weekends, holidays or on-call required!
Competitive pay with mileage reimbursement
Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.
The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
