Home Health Registered Nurse (RN)
Location: Within a 60‑mile radius of 1501 S Yale St., Suite 101, Flagstaff, AZ 86001
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Weekly Guarantee: 36 hours
Productivity Expectation: 30 points
On‑Call Requirement: Yes, rotating weekends
Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient
Position Overview
The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes, ensuring high‑quality clinical services, patient safety, and compassionate support. This role requires strong clinical judgment, autonomy, and the ability to manage a field‑based caseload within a 60‑mile service radius.
Key Responsibilities
- Deliver skilled nursing care to patients in the home setting in accordance with the plan of care.
- Perform comprehensive patient assessments, including physical, psychosocial, and environmental evaluations.
- Develop, implement, and revise individualized care plans in collaboration with the interdisciplinary team.
- Provide patient and caregiver education on disease processes, medications, treatments, and self‑management strategies.
- Complete timely and accurate clinical documentation to meet regulatory, agency, and payer requirements.
- Communicate effectively with physicians, care coordinators, therapists, and other team members to ensure continuity of care.
- Participate in rotating on‑call and weekend coverage as assigned.
Required Qualifications
- Active Registered Nurse (RN) license in the state of Arizona.
- Strong clinical assessment and critical‑thinking skills.
- Valid driver’s license, reliable transportation, and ability to travel within the 60‑mile service radius.
