Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system.
Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

    • Caregivers working 16hrs or more per week are eligible for Full Benefits, 401K and PTO accrual
  • Mileage Reimbursement:
    • Currently $0.70/mile (subject to federal rate changes).
  • Education Assistance:
    • ASN to BSN assistance (up to $16,000).

What You’ll Do:

  • Deliver skilled nursing care as directed by a physician in a home setting.
  • Assess patient needs and develop individualized care plans.
  • Monitor patient progress, document services provided, and adapt care as needed.
  • Educate patients and families on condition management, medications, and recovery.
  • Collaborate with physicians and interdisciplinary teams to ensure continuity of care.

✅ What You’ll Need:

  • RN License in Missouri
  • BLS Certification (American Heart Association)
  • Valid Driver’s License & Auto Insurance
  • Preferred: 1 year of experience in home health, acute care, or a related field

Our Mercy health system was founded by the Sisters of Mercy in 1986. But our heritage goes back more than 185 years. It began with an Irish woman named Catherine McAuley, who wanted to help the poor women and children of Dublin. Though Catherine had a modest upbringing, she received an unexpected inheritance that allowed her to fulfill her dreams. In 1827, she opened the first House of Mercy in Dublin, intending to teach skills to poor women and educate children. Many volunteers came to help. A few years later, Catherine founded the Sisters of Mercy, the first religious order not bound to the rules of the cloister, whose Sisters were free to walk among the poor and visit them in their homes. By the time Catherine died in 1841, there were convents in Ireland and England, and in 1843, the Sisters of Mercy came to the United States.
