Home Health RN – Hospice Case Manager

Location – Houston, Texas (Coverage areas will be assigned within the Houston region.)

Schedule: Monday–Friday, 6:00am–5:00pm (8‑hour shift within this window)

Weekly Hour Guarantee: 40 hours

Productivity Expectation: 26 points

On‑Call Requirement: Yes

Mileage Reimbursement: Yes (mileage reimbursed between visits, from first patient to last patient)

Pay Rate: $48/hr - $50/hr

Position Overview

The Hospice RN Case Manager is responsible for coordinating and delivering skilled nursing care to hospice patients in their homes. This role ensures high‑quality clinical care, effective communication, and compassionate support for patients and families.

Key Responsibilities

• Provide comprehensive nursing assessments and ongoing patient care in the home setting
• Develop, implement, and update individualized care plans
• Manage a caseload of hospice patients and ensure timely, appropriate visits
• Educate patients and families on disease progression, symptom management, and hospice philosophy
• Coordinate care with physicians, social workers, chaplains, CNAs, and other team members
• Maintain accurate, timely clinical documentation
• Participate in on‑call rotation as scheduled
• Ensure compliance with agency policies, regulatory standards, and LHC Group GL coding guidance (per LHC Group Contract Labor GL Coding Guide)

Required Qualifications

• Active RN license (Texas)
• Associate Degree in Nursing or higher
• Valid driver’s license and reliable transportation
• Ability to work independently in the field
• Strong clinical judgment and communication skills

Preferred Qualifications

• Hospice experience (highly valued)
• Experience with home health or community‑based nursing
• Strong organizational skills and comfort with productivity expectations
