Develop care plans under primary care providers' orders
Provide direct patient care per the plan of care and scope of practice
Ensure accurate and timely documentation
Participate in QAPI program for top-notch patient care
Coordinate interdisciplinary home health services
Cover weekend and after hours on call on a rotating basis
Qualifications:
Registered Nurse (RN) licensure
Must have prior Home Health agency experience
Must have OASIS knowledge
Current BLS certification from AHA
Licensed driver with an insured automobile in good working order
