Job Summary

The Registered Nurse reports to the Director of Professional Services and is responsible for administering skilled nursing care to clients in their homes. This role involves teaching and supervising the family and other members of the nursing team. Services are performed in accordance with physicians' orders and under the direction and supervision of the Clinical Manager and the Case Manager.

Essential Functions of Position

  • Administer nursing care according to the patient's plan of care and needs.
  • Care for multiple patients throughout the day in their individual home environments.
  • Teach the patient and/or family members about new diagnosis, care, and support skills.
  • Communicate regularly with RN Case Managers, supervisors, and the interdisciplinary care team.
  • Coordinate and deliver nursing care to patients and families.
  • Assess and evaluate patient's level of care, needs, and requirements.
  • Assess the patient's response to the Plan of Care and recommend changes as needed.
  • Evaluate Outcomes of Care.
  • Supervise LPN/LVNs, home health aides, and paraprofessionals providing services to the patient according to regulatory guidelines.
  • Participate in on-call coverage.
  • Submit work in a timely manner and update and maintain patient medical records.
  • Participate in regularly scheduled interdisciplinary team meetings to coordinate care, exchange information, and problem solve.
  • Apply policies and procedures of the agency and the rules and regulations of Federal and State regulatory agencies.
  • Participate in agency quality assessment performance improvement programs.
  • Ensure growth and profitability of the company through the responsible use of company resources and community education.
  • Meet or exceed established productivity standards.
  • Provide education to the patient/family regarding disease process, self-care techniques, prevention strategies, and community resources.
  • Willingly accept direction from Director of Professional Services/Patient Care Manager.
  • Participate in community programs and committees, as requested.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Prepare clinical and progress notes that demonstrate progress toward established goals.
  • Inform the physician and other personnel of changes in the patient's needs and outcomes of intervention.
  • Assess the ability of the caregiver to meet the patient's needs at admission and throughout care.
  • Evaluate patient/family response to care on an ongoing basis.
  • Implement the individualized plan of care and recommend revisions as necessary.
  • Manage discomfort and provide symptom relief and specialized nursing skills related to palliative and end-of-life care.
  • Determine scope and frequency of services needed based on acuity and patient/family needs.
  • Conduct initial and ongoing comprehensive assessment of patient's needs, including Outcome and Assessment Set (OASIS).

Skills

Required Skills & Experience

  • Registered nurses providing skilled intervention must have 2 years' experience as a RN.
  • Proficient using a tablet.
  • Ability to manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention, and family intervention.
  • Ability to work well with an interdisciplinary team.
  • Excellent written and verbal communication skills.
  • Demonstrated problem-solving and decision-making abilities.
  • Ability to develop and maintain rapport with patients, families, and staff.

Schedule Notes

Monday to Friday, 8 AM to 5 PM. Some on-call may be required. 6-8 patients per day. Medicare Home Health visits. Travel radius: 60 miles from the branch.
