Speech Therapist Job Description
The Speech Therapist will provide skilled speech-language pathology services to patients in a home health setting. The ideal candidate will be organized, clinically confident, and committed to delivering high-quality patient-centered care.
Provide skilled speech-language pathology services to patients in their homes
Evaluate patients with speech, language, communication, cognitive, and swallowing disorders
Develop individualized treatment plans based on patient needs, diagnoses, and functional goals
Provide therapeutic interventions designed to improve communication, cognition, and swallowing function
Assess and treat patients with a variety of neurological, medical, and age-related conditions
Educate patients, family members, and caregivers regarding treatment plans, compensatory strategies, and home exercise programs
Recommend appropriate communication strategies and adaptive equipment when clinically indicated
Monitor patient progress and modify treatment plans as needed
Complete accurate and timely clinical documentation in accordance with home health standards
Collaborate with physicians, nurses, occupational therapists, physical therapists, social workers, and other members of the interdisciplinary care team
Participate in care coordination and communicate changes in patient condition as appropriate
Provide patient and caregiver education to promote safety, independence, and carryover of therapeutic strategies
Follow all applicable state, federal, and organizational requirements for home health speech therapy
Maintain a professional and compassionate approach while working independently in patients' homes
Education and Certification Requirements
Master's degree in Speech-Language Pathology from an accredited program
Current Idaho Speech-Language Pathology license or eligibility for Idaho licensure
Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA) preferred/required according to applicable licensing standards
Current ASHA certification preferred
Current CPR certification preferred
Valid driver's license and reliable transportation
Ability to travel to patient homes throughout the assigned service area
Previous home health speech therapy experience preferred
Strong clinical assessment, documentation, communication, and organizational skills
Preferred Qualifications
Experience providing speech therapy in a home health setting
