Home Health Physical Therapist (PT) – Full-Time | 36-Hour Weekly Guarantee
Location: Northeastern Indiana
Coverage Area: Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble, Steuben, Wells, and Whitley Counties
Schedule: Monday–Friday, 8:00 AM–4:30 PM On-Call/Weekend Requirements: None
Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient
Pay Rate: $50-$53/hr
About the Role
We are seeking a dedicated Home Health Physical Therapist to join our team and provide high-quality, patient-centered care in the comfort of patients’ homes. This role offers autonomy, a supportive environment, and the opportunity to make a meaningful impact on patients’ daily lives.
Key Responsibilities
Perform Start of Care (SOC) assessments, comprehensive evaluations, and ongoing treatment for patients eligible for physical therapy in a home health setting.
Develop individualized care plans based on patient needs, goals, and clinical findings.
Deliver therapeutic interventions to improve mobility, strength, balance, and functional independence.
Collaborate with interdisciplinary team members, including nursing, occupational therapy, and case management.
Document all care accurately and promptly in accordance with agency standards and regulatory requirements.
Communicate effectively with patients, families, and caregivers to support education and care plan adherence.
