    Description

    Location: Servicing patients within a 30-mile radius of Pella, IA
    Schedule: Full-Time, Rotating On-Call Requirements

    What You’ll Do
    - Provide skilled nursing care to terminally ill patients in their homes within a 30-mile service area.
    - Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team.
    - Conduct comprehensive assessments, pain and symptom management, and coordinate medication administration.
    - Educate families on disease progression, hospice philosophy, and hands-on caregiving techniques.
    - Act as a liaison between physicians, social workers, chaplains, and caregivers to ensure seamless care.
    - Participate in on-call rotation, providing after-hours support and emergency visits as needed.
     
    What We’re Looking For 
    - Licensure: Current Registered Nurse (RN) license in the state of Iowa
    - Experience: Hospice or Palliative care experience preferred
    - Certification: BLS required; CHPN a plus
    - Skills: Compassionate communication, clinical expertise, autonomy, and strong organizational skills
    - Other: Valid driver’s license, reliable vehicle, and willingness to travel within a 30-mile radius
