Overview
We are seeking a Full-Time Hospice Social Worker (MSW) for a very flexible Monday–Friday position in the hospice home health setting. This is a field-based role where you’ll work one-on-one with patients and families, helping provide emotional support, community resources, and compassionate end-of-life care.

What You’ll Do:
- Perform psychosocial assessments for hospice patients and families/caregivers in a timely manner
- Help ensure patient and family needs are addressed within the plan of care
- Provide counseling and emotional support related to stress, coping difficulties, grief, and end-of-life care
- Coordinate community resources and assist with applications for Supplemental Security Income, Medicaid, Hospice, and Medicaid Waiver programs
- Serve as a liaison between patients/families and community agencies
- Maintain accurate and timely documentation
- Collaborate with the interdisciplinary hospice team to provide exceptional patient care and customer service

Qualifications:
- Master’s Degree in Social Work required
- Current New Mexico Social Work License required (LMSW or LCSW)
- Valid driver’s license and proof of insurance required
