Schedule: Full-time | 64-80 hours/pay period | Monday to Friday | 8 a.m. to 4:30 p.m.
Pay Range: $35.34 - $46.00 / hour (non-exempt), based on experience, qualifications, and location.
Territory: Must be available to provide services in our West St. Paul territory
As an Admission Nurse, you play a vital role in guiding patients and their families through the admission process, ensuring a seamless transition into care. You’ll assess patient needs, complete thorough evaluations, and serve as a compassionate resource for those making critical healthcare decisions.
As an Admission Nurse, you will:
- Conduct Patient Assessments – Perform thorough evaluations upon admission, documenting all necessary information in compliance with Medicare regulations.
- Manage Patient & Family Relationships – Provide compassionate guidance, educate families on care options, and ensure admission requirements are met.
- Facilitate Admission Process – Complete necessary paperwork, coordinate with the interdisciplinary team, and ensure a smooth transition for patients.
- Build and Maintain Referral Partnerships – Work closely with referral sources to identify potential patients and promote Vivie’s services.
- Other Duties as Assigned – Report critical patient information to the care team to enhance quality care planning and continuity. The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
This job also requires:
- Registered Nurse (RN) license in the state of Minnesota.
- CPR certification is required within six months of hire.
- Valid driver's license and use of an insured personal vehicle.
- Ability to pass state mandated background check.
- Physical capability to perform all essential job functions.
- Ability to read, write, and speak English to ensure effective communication with staff, residents, and families.
