McLeod Hospice is seeking a dedicated Hospice Admissions Liaison to serve as a key resource for patients, families, referral partners, and healthcare providers throughout the hospice admissions process.

Position Summary

The Hospice Admissions Liaison serves as the primary point of contact for hospice referrals and admissions, providing education, guidance, and support to patients and families considering hospice services. This role coordinates the admission process from referral through enrollment, ensuring a seamless transition into hospice care while maintaining exceptional customer service and fostering strong relationships with referral sources.

Key Responsibilities

• Receive and process hospice referrals from hospitals, physicians, skilled nursing facilities, and community sources.
• Meet with patients and families, either in person or virtually, to explain hospice philosophy, services, eligibility requirements, and plans of care in a compassionate and understandable manner.
• Provide emotional support and answer questions to assist families in making informed end-of-life care decisions.
• Facilitate hospice enrollment and election processes, including obtaining required consents, signatures, and documentation.
• Complete and submit admission paperwork accurately and efficiently.
• Communicate admission details promptly with the interdisciplinary clinical team to ensure timely initiation of hospice services.
• Coordinate with intake, scheduling, and clinical leadership teams to support smooth transitions into care.
• Identify and escalate clinical concerns appropriately to licensed clinical staff.
• Serve as the hospice admissions liaison to acute care facilities, including hospitals, emergency departments, and inpatient units.
• Build and maintain strong professional relationships with case managers, social workers, discharge planners, physicians, and other referral partners.
• Educate referral sources on hospice services, eligibility criteria, and admission processes.
• Ensure compliance with federal, state, and organizational hospice regulations, policies, and standards.
• Participate in quality improvement initiatives, audits, and performance improvement activities related to admissions and referral management.

Work Schedule:

• 80 hours bi-weekly
• Day Shift
• Rotating weekend coverage required
• 8:00AM - 5:00PM

Qualifications:

Experience & Training

• Experience in healthcare admissions, patient access, care coordination, social services, healthcare sales, or liaison-related roles preferred.
• Strong communication and interpersonal skills with the ability to discuss end-of-life care sensitively and professionally.
• Excellent organizational skills and attention to detail.
• Ability to work independently while collaborating effectively with interdisciplinary healthcare teams.
• Valid driver''s license and reliable transportation for field-based responsibilities.
• Current South Carolina driver''s license and automobile insurance as required by law.

Education

• Bachelor''s degree in a related field preferred.
• Associate degree with relevant experience may be considered in lieu of a bachelor''s degree.
