Position Summary

The Admissions RN is responsible for coordinating and processing hospice referrals to ensure timely and seamless patient admissions. This role serves as a key clinical resource for referral intake, reviewing and accepting referrals from various sources via phone, fax, and electronic systems. The Admissions RN collaborates closely with referral sources, physicians, intake staff, and the clinical team to coordinate initial hospice assessments and ensure compliance with hospice regulations and organizational standards.

Key Responsibilities

  • Receive, review, and prioritize hospice referrals to support timely admissions and continuity of care

  • Respond promptly and professionally to referral inquiries and intake calls

  • Enter and maintain accurate patient demographic, clinical, and insurance information within the EMR

  • Obtain physician orders, document Certification of Terminal Illness (CTI) information, and ensure referral documentation is complete and compliant

  • Coordinate admission scheduling and communication with the clinical team, Patient Care Supervisors, and Patient Care Coordinators

  • Review and co-sign referrals entered by LVNs and non-clinical intake staff as required

  • Collaborate with billing, insurance verification, pharmacy, and PBM partners to facilitate smooth patient onboarding

  • Apply Medicare, state, and hospice regulatory guidelines when reviewing and processing referrals

  • Maintain accurate, timely documentation and support efficient workflow within the intake and admissions process

  • Perform additional duties as assigned

Qualifications

  • Active Registered Nurse (RN) license in applicable state required

  • Minimum 1 year of RN experience required

  • Hospice, home health, or admissions/intake experience preferred

  • Strong clinical judgment, organizational, and communication skills

  • Experience working within EMR systems and managing multiple priorities in a fast-paced environment

  • BLS/CPR certification required within 30 days of hire

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Routine use of standard medical equipment and tools associated with clinical care is essential.
