HOSPICE ADMISSIONS REGISTERED NURSE

POSITION OVERVIEW
The Hospice Admissions Registered Nurse serves as the initial clinical point of contact for prospective hospice patients and their families throughout the Columbus-Ohio area.
In this role, you will evaluate patients for hospice eligibility, educate patients and families on hospice services and benefits, complete the admission process, and develop an individualized plan of care that ensures a seamless transition into hospice services.
The Admissions RN partners closely with physicians, referral sources, clinical leadership, and the interdisciplinary hospice team to deliver a timely, compassionate, and coordinated admission experience.
KEY RESPONSIBILITIES
- Evaluate prospective patients to determine hospice eligibility.
- Review diagnoses, medical history, current symptoms, functional status, and supporting clinical documentation.
- Communicate effectively with physicians, referral sources, patients, families, and caregivers throughout the admission process.
- Educate patients and families on hospice philosophy, services, Medicare benefits, levels of care, and expectations.
- Guide patients and families through emotionally sensitive conversations with empathy, professionalism, and compassion.
- Complete all admission assessments, consents, clinical documentation, and required records accurately and efficiently.
- Develop individualized plans of care based on each patient's clinical condition, goals, symptoms, and immediate needs.
- Coordinate medications, durable medical equipment, supplies, and additional services to ensure a smooth transition into hospice care.
- Collaborate with the Clinical Manager and interdisciplinary team to facilitate seamless patient admissions.
- Communicate significant clinical findings and changes in patient condition to the appropriate team members.
- Maintain timely and accurate documentation in compliance with company policies, Medicare regulations, and all applicable standards.
- Independently manage daily admission responsibilities while maintaining effective communication with leadership.
- Participate in limited on-call responsibilities as needed.
- Perform additional clinical or administrative duties as assigned.

- Minimum of one year of hospice nursing experience.
- Thorough understanding of hospice philosophy, Medicare eligibility requirements, hospice benefits, and levels of care.

- Strong clinical assessment, critical-thinking, and decision-making skills.

REQUIRED QUALIFICATIONS
- Current, unrestricted Ohio Registered Nurse license.
- Minimum of one year of hospice nursing experience.
- Working knowledge of hospice eligibility requirements and the hospice admission process.
- Valid driver's license.
- Reliable personal transportation.
- Current automobile insurance.
- Ability to travel throughout the assigned Dallas–Fort Worth territory.
- Willingness to participate in limited on-call responsibilities.
SCHEDULE
- Full-time
- Flexible schedule based on patient admission needs
- Local travel throughout Franklin County
COMPENSATION
- Base Salary: $85,000–$90,000 annually, based on experience
- Mileage Reimbursement: $0.73 per mile
