We are looking for an experienced, talented, and compassionate Full Time Hospice Admissions Registered Nurse to join our team!

How You'll Contribute: 

The Hospice Admissions Registered Nurse willsupport the Hospice Clinical Managers, triage and business development team in the coordination and scheduling of Hospice admissions and follow up visits as needed. Assist in the communication and coordination of quality services of the mufti-disciplinary team for safe and exceptional patient/client care.

  • Develop, implement, and evaluate individualized, outcome-oriented patient care plans; engage patients and families in care planning and complete all required documentation per agency policy.

  • Conduct start-of-care visits; initiate, update, and manage plans of care, addressing all identified problems or documenting rationale when not addressed.

  • Perform diagnostic, preventive, and rehabilitative nursing procedures; assess ongoing client needs and communicate condition changes to physicians and appropriate healthcare professionals.

  • Educate, supervise, and counsel clients and families on home care procedures, infection control, and safety measures; ensure proper documentation.

  • Oversee and revise home health aide services and unskilled care plans; report care plan changes to the Clinical Manager as needed.

  • Coordinate multidisciplinary care plans, collaborate with healthcare teams, and lead or participate in case conferences for complex clients.

  • Maintain accurate, timely, and compliant clinical records; ensure adherence to federal, state, and agency regulations and corporate compliance standards.

  • Demonstrate strong clinical judgment, problem-solving, client focus, relationship management, cultural competence, flexibility, and commitment to ongoing professional development.

Schedule: Monday - Friday 9:00 am-5:00 pm 

Compensation: 42/hr. - 57/hr. Based on experience

What Makes You a Great Fit:

  • 2+ years of Registered Nurse experience 
  • Hospice experience strongly preferred
  • Strong clinical assessment skills
  • Strong computer skills
  • Active Registered Nurse license in MA
