As a leader in hospice care, we focus on enhancing quality of life by offering physical, emotional, and spiritual support. We are currently seeking a skilled and compassionate Bereavement & Volunteer Coordinator to join our hospice team. This role is essential in ensuring families receive ongoing emotional support after the loss of a loved one while also building and leading a strong volunteer program that enhances the care we provide to patients and families.

What You'll Do

Bereavement Coordination

  • Manage the hospice bereavement program in compliance with Medicare Conditions of Participation.

  • Provide grief support to families for up to 13 months following a patient’s death.

  • Conduct bereavement assessments to identify individuals at risk for complicated grief.

  • Coordinate grief counseling, support groups, memorial services, and follow-up communications.

  • Maintain accurate documentation and bereavement care plans in the EMR.

  • Work closely with the interdisciplinary team (IDT) to ensure holistic family support.

Volunteer Program Leadership

  • Recruit, screen, and onboard hospice volunteers.

  • Provide training, supervision, and ongoing support for volunteers.

  • Match volunteers with appropriate patient, family, and administrative opportunities.

  • Maintain compliance with volunteer documentation and training requirements.

  • Track and report volunteer hours and program effectiveness.

  • Develop community partnerships to expand the volunteer network.

Qualifications

  • Bachelor’s degree in Social Work, Counseling, Psychology, or related field preferred

  • Experience in hospice, grief counseling, or bereavement services preferred

  • Experience managing or coordinating volunteer programs preferred

  • Excellent interpersonal and communication skills

  • Strong organizational and documentation skills

  • Ability to work compassionately with patients and families experiencing loss

  • Valid driver’s license and reliable transportation
