We are seeking a compassionate, organized, and experienced Bereavement Coordinator to join our hospice team.
In this role, you will coordinate and provide grief and bereavement support services to families and caregivers, helping them navigate the emotional challenges associated with end-of-life care and the loss of a loved one.
This is a full-time hybrid position offering a combination of remote administrative responsibilities and in-person family support, community outreach, and team collaboration.

Key Responsibilities
As a Bereavement Coordinator, you will be responsible for developing, coordinating, and delivering bereavement services that support patients, families, and caregivers throughout the grieving process.
Responsibilities Include:
- Provide emotional support, grief counseling, and bereavement education to patients, families, and caregivers.
- Assess bereavement needs and develop individualized support plans.
- Coordinate and provide bereavement follow-up services for a minimum of 13 months following a patient's death.
- Facilitate individual grief support, bereavement groups, and educational programs.
- Plan and coordinate memorial services, remembrance events, and family support activities.
- Identify individuals experiencing complicated grief and coordinate appropriate referrals to mental health professionals and community resources.
- Maintain accurate and timely documentation of bereavement assessments, services, and follow-up activities.
- Collaborate with hospice nurses, social workers, chaplains, volunteers, and other interdisciplinary team members.
- Assist with coordinating and training bereavement volunteers.
- Provide grief education and support to hospice staff as needed.
- Ensure bereavement services comply with hospice regulations, organizational policies, and professional standards.
- Participate in interdisciplinary team meetings and occasional evening or weekend responsibilities as needed.
Qualifications & Requirements
Education & Training:
- Bachelor's degree in Social Work, Counseling, Psychology, Theology, Human Services, or a closely related field, or specialized certification in bereavement counseling. Master's degree preferred.
- Demonstrated education or formal training in grief, loss, bereavement, death and dying, or end-of-life care.
Applicants must be able to provide documentation of relevant grief and bereavement education or training, such as:
- Graduate transcript showing coursework in grief, loss, bereavement, death/dying, hospice/palliative care, or closely related counseling.
- Continuing Education (CE) certificate or documentation of formal grief/bereavement training.
Experience & Skills:
- Previous experience in grief counseling, bereavement coordination, social work, counseling, or providing emotional support to individuals and families experiencing loss.
- Hospice, palliative care, or end-of-life care experience preferred.
- Strong knowledge of grief reactions, bereavement support, and community resources.
- Ability to recognize signs of complicated grief and make appropriate referrals.
- Excellent communication, interpersonal, and active listening skills.
- Strong organizational, documentation, and time-management abilities.
- Ability to manage responsibilities independently while working collaboratively with an interdisciplinary healthcare team.
- Compassionate, professional, and sensitive approach when working with grieving families.
- Proficiency with electronic documentation and basic computer applications.
- Valid driver's license, reliable transportation, and willingness to travel locally as needed.

This position combines remote administrative work with in-person responsibilities, including:
- Family visits and bereavement support

Local travel is required based on family needs and program responsibilities.
