Job Summary
Unified Care Hospice is seeking a compassionate and dependable Bilingual Certified Nursing Assistant (CNA) to join our growing clinical team. In this role, you will provide hands-on personal care and emotional support to patients and families in their homes and care facilities. Positions are available for Full-Time, Part-Time, and PRN (as-needed) schedules.
Bilingual (English/Spanish) candidates are strongly preferred to better serve our diverse community.
Employment Status Options
- Full-Time: Regularly scheduled hours with benefits eligibility
- Part-Time: Consistent, limited hours based on availability and census
- PRN (As Needed): Flexible scheduling based on patient needs; no guaranteed hours
Responsibilities
- Provide personal care including bathing, grooming, dressing, toileting, and feeding
- Support patient comfort, safety, and dignity at all times
- Monitor and document patient condition and vital signs in the EMR
- Follow the individualized Plan of Care under RN supervision
- Assist with safe transfers and mobility using appropriate equipment
- Maintain infection control and safety standards
- Communicate clearly with patients, families, nurses, and leadership
- Complete daily/same-day charting per agency policy
- Deliver supplies and assist with basic patient support as needed
- Participate in required trainings and in-service education
Qualifications & Experience
- Active CNA certification in the State of Texas
- Current CPR certification
- Bilingual (English/Spanish) preferred
- Valid driver’s license, reliable transportation, and active auto insurance
- Prior hospice, home health, or long-term care experience preferred
- Familiarity with EMR systems
- Strong professionalism, reliability, and time management skills
- Ability to work independently and within an interdisciplinary team
- Experience with dementia, behavioral health, or end-of-life care is a plus
Physical Requirements
- Ability to lift, reposition, and assist patients safely
- Ability to stand, walk, bend, and stoop for extended periods
- Ability to travel to patient homes and facilities
