The Music Therapist is responsible for providing music therapy services to hospice patients according to their plan of care. Duties include but are not limited to:

• Perform assessments, evaluations, and establishes a plan and goals
• Perform music therapy treatments as prescribed by the Attending Physician and the Suncrest Hospice team
• Report on client's response to treatment or any changes in condition
• Document services delivered using electronic methods on day of service
• Participate in interdisciplinary team (IDT) meetings and collaborate with physicians, nurses, chaplains, aides, facility staff, and other members of the care team
• Ability to work well with verbal and non-verbal clients
• Connect with families and staff in a variety of outreach settings
• Ability to improvise in a clinical setting

Qualifications:

• Currently Music Therapist Board Certified (MT-BC)
• One year of music therapy experience
• Completed Music Therapy Degree Requirements
• Must be willing to drive with reliable transportation, valid driver's license and required vehicle insurance
