Hospice Care Liaison/Sales - Collin County.

The Care Liaison will be responsible for building and maintaining relationships with referral sources, including hospitals, physician offices, nursing facilities, and community organizations, to drive patient referrals and expand our hospice services in Collin County. This role requires a compassionate, results-driven professional with strong interpersonal skills and knowledge of the healthcare industry.
The primary functions of this role include:
- Develop and maintain relationships with healthcare providers, including physicians, hospital discharge planners, and case managers, to generate hospice referrals.
- Educate referral sources and the community about the benefits of hospice care and our services.
- Conduct outreach through in-person visits, presentations, and networking events to promote Hope Health Care.
- Collaborate with the clinical team to ensure seamless transitions for patients entering hospice care.
- Meet or exceed referral and admission goals through strategic marketing and relationship-building efforts.
- Maintain accurate records of outreach activities and referral data using CRM software.
- Stay informed about industry trends, competitor activities, and regulatory requirements related to hospice care.
- Demonstrate a comprehensive understanding and ensure compliance with local, state, and federal laws relating to the promotion of the agency.
Qualifications and Skills:
- Bachelor’s degree in marketing, business, healthcare administration, or a related field (preferred but not required).
- Minimum of 2 years of successful and scalable sales experience in hospice.
- Proven track record of building relationships with healthcare professionals and achieving sales goals.
- Strong understanding of hospice care principles and philosophy.
- Excellent communication, presentation, and interpersonal skills.
- Ability to work independently, manage time effectively, and prioritize tasks.
- Valid driver’s license, reliable transportation, and auto insurance (required).
- Proficiency with CRM software and Microsoft Office Suite.
- Compassionate, empathetic, and professional demeanor.
- RN, LVN or social work license a plus.
