We’re hiring a Hospice Case Manager (RN) who wants meaningful one-on-one patient relationships, schedule stability, and the opportunity to provide compassionate end-of-life care while maintaining an excellent work-life balance.

You’ll step into a field-based hospice role providing direct patient care in homes and care facilities throughout Cape Cod. You’ll independently manage your patient caseload while collaborating with an experienced interdisciplinary team dedicated to providing exceptional end-of-life care. Your territory will include Barnstable, Bourne, Cape Heritage, Cape Regency, and surrounding Cape Cod communities.

• Manageable census of approximately 20 patients
• Additional compensation for rotating weekend on-call coverage

What makes you a great fit

• You hold an active Massachusetts RN license
• You have at least one year of nursing experience
• Hospice, home health, ICU, or oncology experience is preferred
• You value compassionate, patient-centered end-of-life care
• You’re comfortable independently managing a field-based caseload
• You work well as part of an interdisciplinary healthcare team
• You’re comfortable traveling throughout the Cape Cod territory

Compensation & Benefits

• Base salary: $104,000–$114,400 annually
• Weekend on-call: $600 per weekend
• Routine on-call visits: Additional $100 per visit
• On-call admissions: Additional $150 per admission
• Health insurance: Employer pays 60% of employee premium
• Dental insurance: Employer pays 60% of employee premium
