As an RN Case Manager, you'll have the opportunity to make a meaningful difference by helping patients and families navigate their care with confidence and support. This role combines hands-on nursing expertise with care coordination, advocacy, education, and collaboration to ensure patients receive the right care at the right time.
RN Case Managers serve as a central connection between patients, families, nurses, physicians, social workers, aides, community agencies, and other members of the interdisciplinary team. You'll help coordinate individualized plans of care while providing clinical guidance and supervision to LPNs, aides, and student nurses involved in the patient's care.
